Position Summary
Coordinates health care system outreach and education for the community. Administers programs to develop community relationships with healthcare providers. Drives health service awareness to assist members in maintaining optimal health outcomes.

What you will do

Executes both routine and non-routine business support tasks for the Community Health area under limited supervision, referring deviations from standard practices to managers.Follows area protocols, standards, and policies to provide effective and timely support.Builds and maintains effective working relationships with members, providers, case managers, and agency representatives, and assists with care plan implementation.Manages initiatives to reduce culture and socio-economic barriers between members and providers and identifies gaps and opportunities to strengthen systems of care with the community.Maintains member health records, compiles reports, develops educational materials, and completes program documentation.Takes direction to execute techniques, processes, and responsibilities.

Required Qualifications

Minimum of 1 year of relevant experience, such as community outreach or conducting home visits with members.Working knowledge of problem-solving and decision-making skills.At least 1 year of experience working with Medicaid.Willingness to travel within a 50-mile radius in the Maryland area, up to 70% of the time.

Preferred Qualifications

Experience working in a community or outpatient setting.Community Health Worker certification or equivalent training and experience.Preferred: Specialized training in medical terminology and practices related to designated health conditions.Proficient in computer use, including Excel and Electronic Health Records (EHR) systems.

Education

High School Diploma required and Associates degree in a healthcare field (preferred).

Pay Range

The typical pay range for this role is:

$18.50 - $38.82

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
