Position Purpose:
Mercy Care Atlanta is seeking a compassionate and community-driven Community Health Worker (CHW) to support Spanish-speaking and non-Spanish-speaking clients. This role is vital in empowering individuals and communities by increasing health knowledge, promoting self-sufficiency, and improving access to care.
What You Will Do:
As a CHW, you will:
Provide outreach, education, informal counseling, and advocacy.
Conduct home visits and assessments to identify barriers to care.
Link clients to health and social services.
Educate individuals on chronic conditions and health promotion.
Serve as a liaison between patients and providers.
Document services and outcomes in EPIC or designated platforms.
Collaborate with community agencies and clinical teams.
Participate in community meetings and expand knowledge of local resources.
Minimum Qualifications:
High School diploma or GED (or equivalent in country of origin).
At least one year of experience working with underserved populations.
Understanding of community resources.
Ability to legally drive in Georgia (valid GA driver’s license may be required).
Bilingual (Spanish-English) preferred but not required.
