**Description:**
**Position Purpose:**
Mercy Care Atlanta is seeking a compassionate and community-driven **Community Health Worker (CHW)** to support Spanish-speaking and non-Spanish-speaking clients. This role is vital in empowering individuals and communities by increasing health knowledge, promoting self-sufficiency, and improving access to care.
**What You Will Do:**
As a CHW, you will:
+ Provide outreach, education, informal counseling, and advocacy.
+ Conduct home visits and assessments to identify barriers to care.
+ Link clients to health and social services.
+ Educate individuals on chronic conditions and health promotion.
+ Serve as a liaison between patients and providers.
+ Document services and outcomes in EPIC or designated platforms.
+ Collaborate with community agencies and clinical teams.
+ Participate in community meetings and expand knowledge of local resources.
**Minimum Qualifications:**
+ High School diploma or GED (or equivalent in country of origin).
+ At least one year of experience working with underserved populations.
+ Understanding of community resources.
+ Ability to legally drive in Georgia (valid GA driver’s license may be required).
+ Bilingual (Spanish-English) preferred but not required.
