As a Community Health Worker, you will be assessing, planning,  implementing, monitoring, and evaluating actions to meet with clients health and human services needs, so that they can adopt health behaviors leading to self-sufficiency. You will be coordinating and providing client support that is safe, timely, effective, efficient, equitable, and client centered.

Requirements for working as a Community Health Worker   

High School Diploma or equivalent.Bachelor’s degree in a human services field.Any combination of experience with health, education, and social services for a minimum of 1 year.  Basic understanding of health conditions, such as HIV/AIDS, Diabetes, COPD, Asthma, CHF, CVDBilingual Spanish preferred.

What Community Health Workers will do:

Communicating to clients the purposes of the program and showing them, the impact it may have on them.Conducting intake interviews, enrolling and or referring individuals into community programs.Assisting health care providers involved in the clients care, to ensure clients understanding of care plans.Supporting the development, implantation, and execution of care plans.Monthly reporting, maintaining electronic client files, including updated release of information, assessments, and other medical documents.Travels extensively to client homes, community locations, agencies, and other outreach destinations.
