Community Health & Wellness Manager
Job Title: Community Health & Wellness ManagerDepartment: Community Health & WellnessLocation: Alexander ParkHours Per Week: 40 hours per weekSchedule: Monday - Friday; Days SUMMARY: The position holds a pivotal role in promoting health and well-being within the community. This role is responsible for planning, coordinating and implementing wellness programs and activities to educate and engage individuals in healthier lifestyles and improve health equity. KEY RESPONSIBILITIES: + Developing and implementing outreach strategies to promote wellness programs. + Organizing health screenings, workshops, and other educational events. + Collaborating with community leaders, organizations, and health care providers to promote wellness initiatives. + Assessing the needs of the community and identifying health and wellness issues. + Providing resources and support to individuals to help them achieve their health and wellness goals. + Collecting and analyzing data to evaluate the effectiveness of wellness programs. + Creating and distributing health education materials. + Ensuring all programs comply with local, state, and federal regulations. + Keeping up to date with the latest research and trends in community health and wellness. + Facilitate workshops, seminars, and other educational events focused on health and wellness. + Collaborate with healthcare professionals, community leaders, and other stakeholders to address community health needs. + Develop and maintain relationships with community members and groups to encourage participation in wellness programs. + Track and report on program outcomes and participant feedback. + Assist with community benefit reporting and sponsorship investment tracking to support community engagement and social impact agenda. + Stay informed about current trends and developments in community health and wellness. + Comply with all applicable regulations and guidelines. + This position may involve working in both office and community settings. Some travel may be required for outreach events and meetings. PREFERRED QUALIFICATIONS: + Proven experience as a Community Wellness Coordinator or similar role in health education or community outreach. + Strong knowledge of health promotion strategies and best practices. + Excellent organizational and project management skills. + Ability to work with diverse populations and understand community health needs. + Strong communication and interpersonal skills. + Thorough knowledge of principles and practices of community outreach and engagement strategies. + Working knowledge of methods and techniques of effective outreach to underserved populations. + Working knowledge of the principles and practices of program and project management. + Working knowledge of and proficiency with Windows-based PC systems and Microsoft suite. + Ability to foster effective working relationships, influence others and build consensus with individuals at all levels in the organization. + Ability to demonstrate a collaborative management style, build rapport, and effectively develop and manage internal and external business relationships + Ability to adapt to or initiate change. + Ability to work independently and make decisions related to areas of functional responsibility. + Ability to bend, lift, and carry objects of varying sizes weighing up to 10 pounds. + Ability to work effectively in a remote work environment. + Ability to travel to different locations in the course of work. + 2+ years of experience related to community outreach and engagement strategies preferred. + Bachelor’s degree in Public Health, Health Education, or a related field preferred. REQUIRED QUALIFICATIONS: + Associate degree in Public Health, Human Services, Social Science, Social Welfare, Psychology, Healthcare Administration, or a related field required. + 6+ six years of experience in community health education, community relations, public relations, or public assistance programs required. EDUCATION: LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. PAY RANGE: $70,000.00 - $85,000.00 CITY: Rochester POSTAL CODE: 14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
