Serves as the major tertiary facility for much of the Southeast, the Caribbean and South America. AdventHealth Orlando houses one of the largest Emergency Departments and largest cardiac catheterization labs in the country. We are already one of the busiest hospitals in the nation, providing service excellence to more than 32,000 inpatients and 125,000 outpatients each year **Job Summary:** The **Community Health** team will serve as the Divisional leaders striving to improve the health of the communities we serve. The team will ensure Community Benefit requirements are met as outlined by the IRS, as well as lead efforts to develop, implement and evaluate strategic health improvement efforts in the community. The program manager position is responsible for executing Community Health strategies in their assigned area of responsibility as well as serving as the content expert for specific functions for the Central Florida Division Community Health team. The program manager is responsible for execution of IRS-required functions including, but not limited to: Community Health Needs Assessment (CHNA) Community Health Plan development, implementation, and evaluation Ongoing tracking of Community Benefit In addition, the program manager will serve as the content expert for specific functions for the Community Health team. This will include oversight of process development and implementation as assigned. The CHNA Program Manager will be responsible for serving as the program lead for the CHNA which is a 1218-month collaborative process comprised of collecting primary and secondary data, performing data analysis, priority selection and completion of a CHNA report every three years. In addition to leading the divisional strategy for the CHNA, this program manager will focus on ensuring all Community Health functions comply with the most updated IRS regulations to ensure maintenance of non-profit status. This role will also serve as the research lead to identify best practice community health initiatives on both a state and national level. All team members will actively participate in outstanding customer service and accept responsibility in maintaining relationships that are equally respectful to all. **PRINCIPAL DUTIES AND JOB RESPONSIBILITIES** **:** **Community Health Team Members all contribute to:** Analyze the health care environment to identify and lead opportunities for the advancement of community benefit by assigned coverage area Advocate for health improvement efforts through policy, systemic and environmental change. **For assigned campuses/coverage area program managers all contribute to:** **Campus Engagement** Work with campus leaders to create and execute CHNA, CHP, complete annual evaluation of CHP, and serve as campus point of contact for updates on progress to ensure inclusion of community benefit within strategic and financial planning processes Inform hospital CEO, campus administrators and other top executives on Community Health initiatives, progress, and new requests through quarterly meetings by assigned coverage area **Program Implementation** Collects metric information for strategic health programs for assigned coverage area. Compiles various department reports and assists department management with budgetary oversight for assigned coverage area Support the effectiveness and efficiency of reporting of community benefit initiatives through improvements of coordination and communication between support and business functions. **Community Engagement** Establish, maintain, and grow relationships with key partners and organizational leaders to further community health programming and objectives for assigned coverage area Lead collaborative, interdisciplinary teams and committees related to CHNA and CHP development and implementation Partner with Community Relations to establish and grow relationships with key partners and organizational leaders to further community health programming and objectives in assigned coverage area **The CHNA Program Manager oversees:** **Community Health Needs Assessment** Provides strategic direction, development, planning, execution, and analysis in all aspects of the Community Health Needs Assessment process in order to meet regulatory requirements and organizational and departmental goals and objectives Associates population health improvement with community benefit by identifying best practices to improve quality of life and reduce health disparities in the community, based upon the needs found in the Community Health Needs Assessment (CHNA) **Compliance/Regulatory** Serve as a liaison to legal for partnerships, licensing, and other contracts. Maintains a working knowledge of applicable Federal, State, and local laws to ensure adherence and compliance with community benefit regulations and guidelines. Leads team in timely and accurate compliance with state and federal regulatory guidelines and standards including Community Health Needs Assessments (CHNAs), Implementation Strategies, annual state Community Benefit Reports [if applicable], and IRS Schedule H narrative responses. **Community Health Research** Associates population health improvement with community benefit by identifying best practices to improve quality of life and reduce health disparities in the community, based upon the needs found in the Community Health Needs Assessment (CHNA) Integrates strategic planning methodologies, and benchmarking and performance improvement tools into community benefit program planning, priority setting, and evaluation **The Campus and Community Engagement Program Manager oversees:** **Community Benefit Reporting** Support the effectiveness and efficiency of reporting of community benefit initiatives through improvements of coordination and communication between support and business functions. Provides strategic direction, development, planning, execution, and analysis in all aspects of Community Benefit tracking in order to meet regulatory requirements and maximize organizational reporting of community benefit cash and in-kind donations **Community Health Research** Associates population health improvement with community benefit by identifying best practices to improve quality of life and reduce health disparities in the community, based upon the needs found in the Community Health Needs Assessment (CHNA) Integrates strategic planning methodologies, and benchmarking and performance improvement tools into community benefit program planning, priority setting, and evaluation **Campus and Community Engagement** Develops infographics and tools to proactively communicate program implementation, evaluation and outcomes to stakeholders and community members Manages requests for internal and external communication (infographics, one-sheeters) for the Community Health team **AdventHealth Foundation Grant Support** Researches and identifies grant opportunities based on department goals and strategy needs. Maintains effective, collaborative relationship with AdventHealth Foundation and Research grant writers Manages the evaluation and tracking of state, local, and federal public funding, and foundation funding opportunities **The Community Grant Program Manager oversees: Operations** Lead analysis of outcome data to determine ROI on grant program investments. Manage leadership correspondence, development of PowerPoint presentations for meetings, agenda/minutes, and distribution. Oversee update and maintenance of department web site. Orients approved 3C fund recipients on data reporting processes and ensures reporting deadlines are met. Keeps detailed records of data and budgetary items **Community Health Plans** Serve as content expert and project lead for Community Health Plan development, implementation and annual evaluation. Serve as point of contact for Corporate and ensure timely review and approval of Community Health Plans and annual CHP evaluations **Employee Grant Program** Manages and maintains CFD-South Employee Grant Program, inclusive of but not limited to, vetting of applications, facilitating funding selection process, tracking, and reporting program outcomes, relationship management with applicants and strategy development for communication rollout. Responsible for maintaining accurate metrics related to employee grant program and community health plans for regulatory reporting to the IRS to maintain non-profit status Designs Employee Grant program metrics based on internal data available and desired outcomes Qualifications **What Will You Need:** **Education and Experience Required:** + **Bachelors degree in business, business/healthcare analytics, healthcare administration, non-profit management, healthcare or non-profit finance, healthcare or non-profit law or related field** + **3-years of management experience in healthcare, business, government or non-profit/community organization** + Health programming, charitable grants, healthcare or non-profit law, healthcare or non-profit finance, or community funding experience + Excellent analytical and problem-solving skills + Strong working knowledge of health care delivery systems, health insurance coverage and reform, uncompensated and undercompensated care, health disparities, social determinates of health, community organizations, Triple Aim and population health management concepts + Advanced knowledge of latest computing software including, but not limited to, Microsoft Office program: Outlook, Word, Excel, and Power Point This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
