Community Health Manager
The Community Health Manager will be responsible for all community health program design for Mt. Ascutney Hospital, development and implementation; community assessment activities; program reporting and improvement recommendations.Thisposition will serve as the Blueprint Program Manager for the Windsor Health Service Area (HSA) in addition to being the facilitator for other organizational community-facing programs, including the Community Collaborative and the Community Health Implementation Plan (CHIP) workgroups.This position will be responsible for completingall applications, renewals, and reporting requirements for federal, state, and foundation-related grants across the community health department. Post procurement, they will be responsible for overseeing logistics and coordination — including efficient communication and reporting — assuring compliance with regulations, adhering to grantor requirements, communicating with both grantor and any subrecipients and/or contractors to assess the success of allocated programs and projects.
