SUMMARY:

The Licensed Practical Nurse (LPN) provides home health services to Rochester Regional Certified Home Health Agency (CHHA) clients under the direction of a Registered Nurse and in compliance with Medicare, Medicaid, and NYS Department of Health regulations. The LPN delivers compassionate, high-quality care while modeling the system’s Brand Promise and Core Values.

RESPONSIBILITIES:

Delivers routine nursing care and education to clients in their homes, observing and documenting signs and symptoms, and reporting health status changes to the supervising Registered Nurse.

Accurately completes timely documentation in accordance with Federal, State, and organizational standards, ensuring quality and compliance across all assigned patient interactions.

Collaborates with Registered Nurses to review and revise care plans as patient needs change, supporting a patient-centered and adaptive care approach.

Participates in on-call, holiday, evening, and weekend rotations to support agency coverage and continuity of care.

Maintains clinical competency and participates in ongoing service education programs to remain current in home health practices and available community resources.

Demonstrates strong critical thinking, professional judgment, and compassionate care in all patient and family interactions.

Performs additional duties as assigned, supporting agency operations and patient needs across the home health continuum.
 

REQUIRED QUALIFICATIONS:

Graduate from an accredited School of Practical Nursing

NYS Licensed Practical Nurse (LPN)

Valid New York State Driver’s License and ability to pass a driver’s license record background check

Basic Life Support (BLS) certification

Strong documentation, organizational, and communication skills

Ability to apply critical thinking and sound clinical judgment in diverse home care situations

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
