RWJBarnabas Health is seeking a highly dedicated Community Health Educator to work in the Community Health Education department, at Monmouth Medical Center Southern Campus.
Monmouth Medical Center Southern Campus is a fully accredited acute care hospital dedicated to providing the finest medical and health care services to the residents of Ocean and Monmouth counties. An affiliate of the RWJBarnabas Health system, Monmouth South is a sister hospital to Monmouth Medical Center in Long Branch.
Monmouth South is the only hospital in the region to provide private rooms on all of its inpatient units, providing an environment more conducive to healing. Several other enhancements have been made to provide superior service to Lakewood and surrounding region as well including the Better Health Senior Membership Program, state-of-the-art Geriatrics Institute, and an expansion of cancer services.

Qualifications:
Required:
Bachelor s degree in a related clinical field and previous experience working with the geriatric population in a community outreach or inpatient setting. Must be a graduate from an accredited school of nursing,Current Registered Nurse License in the state of New JerseyMinimum of 2 years general practice experienceStrong communication and organizational skillsProficient computer skillsExperience with Microsoft Word, PowerPoint, and Excel,Certifications and Licenses Required:
Active New Jersey Registered Nurse LicensePreferred:
BSN BLS AHA certificationBilingual English/Spanish strongly preferredExperience in community outreach preferred,Scheduling Requirements:
Three 8 hour shifts Requires some evenings and weekendsThe Community Health Educator is a Registered Nurse, RN, who provides culturally appropriate, evidence-based health education, obesity prevention, and chronic disease management programs/initiatives for people across the whole continuum of life in Monmouth and Ocean counties,
They organize and facilitate educational programs at MMC and MMCSC, the LiveWell Center in Eatontown, and other community sites,
This position is based out of the LiveWell Center in Eatontown, NJ, deploying to community locations in Monmouth and Ocean County as the programming schedule dictates,
The individual will work one day a week at Monmouth Medical Center Southern Campus in Lakewood,
Essential Functions:
Develops educational programs to address chronic health conditions across the lifespan using data and best-practice information,Presents educational programs to the community, provides or schedules for the provision of programs,Conducts health screenings such as blood pressures, bone density, balance, etc, for community members, obtaining consent, maintaining patient privacy, and providing education at conclusion of screening,Tracks attendance and administers surveys to assess knowledge change,Assists with the Better Health Program for older adults,Assists with the Safe Kids Worldwide program,Maintains data and records related to screening, education, and community events,Works cooperatively and collaboratively with other departments to achieve the goals of the department,Representatives the Hospital at community-events with highest integrity,Performs other duties as assigned,Some evenings and weekends required,Benefits and Perks:
