D'Aniello Institute for Veterans and Military Families (IVMF) is seeking Community Care Coordinator for SyracuseServes. This is a full-time position promoting a person-centered holistic care model and coordinated approach to service delivery to support the health and wellbeing of veterans, service members, and their families through accurate, timely, and successful linkages to resources across the Central New York region. SyracuseServes addresses the social determinants of health for service members, veterans, and their families across the country using community-based approaches. Having access to navigation of care and services helps veterans and their families sustain healthy, productive, and economically stable lives following separation from the armed forces.
