Job Summary:
Join our compassionate and collaborative team as a Community Behavioral Health Specialist, where you will play a vital role in helping individuals achieve and sustain recovery from mental illness and substance use disorders. In this role, you will complete initial and annual assessments, develop wellness plans, and provide educational support to clients and their families. As part of a multidisciplinary team, you will work collaboratively to ensure that clients receive the resources they need to thrive in their recovery journey.

Complete initial and annual assessments within the electronic medical record (EMR) for individuals entering ACS services.Develop person-centered and strengths-based wellness plans.Provide educational information and skill building to clients and families regarding daily living skills, symptom management, and communication.Act as a liaison to community and service organizations, assisting clients in accessing necessary resources.Facilitate educational groups utilizing a motivational approach as assigned.Ensure all documentation is completed per agency standards and policies.Participate as an integral part of the ACS team, providing crisis services and case management as needed.Education, Experience, and/or Credential Qualifications:
Master’s degree in Counseling or Social Work required.Minimum of two years of experience working with adults with Serious and Persistent Mental Illness (SPMI) or children with Serious Emotional Disturbance (SED).Must acquire and maintain required training per agency policy and DMH regulations.Additional Qualifications:
Successful completion of background checks including criminal record, driving record, and abuse/neglect checks.Current driver’s license, acceptable driving record, and current auto insurance.Physical Requirements:
Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.Repetitive movements of hands, fingers, and arms for typing and/or writing during work shifts.Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
