William E Christoffersen Salt Lake Veteran's Home is looking for an energetic CNA Coordinator to come work at out amazing facility. $22.20 - 26.01 PR/HR JOB DESCRIPTION NURSING ASSISTANT – CERTIFIED – COORDINATOR Summary/Objective: Responsible for scheduling and daily assignments of all Certified Nursing Assistants (C.N.A.) with the goal of minimizing overtime hours; training of new hires and current staff as needed; covering CNA shifts if required; completion of audits pertaining to charting, environmental rounds, and setting a positive example for team members with focus on promoting a compassionate physical and psychosocial environment for the residents. This position will work closely with the Assistant Director of Nursing (ADON) and the Director of Nursing (DON). Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Coordinator Functions: o Coordinate assignments for nursing assistants. Assure that nursing assistants complete assignments on time, completely and accurately. Supervise prompt response to resident calls for assistance. o Review CNA documentation for accuracy and timeliness on a daily basis. Including the hydration program and meal consumption sheets. Focused Rounds as assigned. o Assist CNAs in identifying resident needs and initiating corrective action within scope of responsibility and/or seeking assistance of a licensed nurse, as appropriate. Participate in the care planning process as assigned. o Coordinate the orientation of new CNAs with supervision from nursing staff. o Assume Quality Assurance duties as assigned. Assist in ongoing in-service process as identified in collaboration with nursing staff and the facility administrator. o Supervise efficient use of resident supplies by nursing assistants. o Assist to ensure infection control measures are properly observed by CNA staff. o Participate in restorative program as directed. Will also assist in coordinating the admission process for all new residents. • Observe and practice safety according to facility policy and procedure: o Able to explain and demonstrate fire/emergency procedures and must participate in any emergency or drill activities. o Properly use and maintain facility equipment, reporting/taking out of service any equipment with breakage or needed repairs. o Show respect for facility property. o Demonstrate and able to explain infection control and isolation procedures. o Demonstrate and able to explain proper body mechanics in all patient handling interactions & processes. o Ensure residents are given correct diet. o Use proper feeding techniques while respecting personal dignity and meeting the residents’ needs for socialization. o Make beds correctly and wrinkle free. o Maintain residents’ rooms in neat, orderly and clutter-free manner. o Support residents’ participation in activity programs by assisting them (when needed) to the activity location; encourages resident participation in activities. o Question and seek clarification of unclear orders. • Promote and maintain positive relationships with co-workers, residents, visitors, volunteers and regulators: o Directly respond, within the scope of practice, to needs and concerns of residents and family members. o Work with the care giving team to identify and resolve resident and family issues. o Promptly respond to call lights and other resident needs. o Treat residents with compassion and dignity. o Establish effective communication/interaction with residents and families. o Ensure residents’ right to privacy (visual and auditory). o Maintain confidentiality of resident and facility records/information. o Show respect for residents’ personal property. o Ensure residents are dressed appropriately, consistent with weather conditions, and according to their wishes. o Recognize a resident’s ability to make choices and support these within the constraints of the care giving environment. o Communicate with members of the care giving team about residents’ personal choices. o Communicate residents’ end of life concerns and wishes to the Charge Nurse. o Be sensitive and appropriately responsive to each resident’s “end of life” wishes. • Perform the following according to policy and procedure: o Take and record temperature, pulse, respiration and blood pressure o Ambulate residents. o Apply proper lifting and transferring techniques. o Provide proper positioning and use of postural supports. o Maintain residents’ positioning, as needed. o Weigh residents on schedule and record weights accurately. o Provide restorative and rehabilitative nursing. o Provide active and passive range of motion. o Implement bowel and bladder program. o Collect certain specimens for cultures pursuant to facility policy and within the scope of duties. • Ensure residents’ comfort while assisting them in achieving their highest practicable level of functioning as follows: o Assist residents with their activities of daily living. o Provide oral and hygiene care pursuant to plan of care. o Provide nail care pursuant to plan of care. o Assist residents with dressing and grooming according to their needs, wishes and desires. o Bathe residents with attention to privacy, respecting dignity and providing a safe and relaxing experience. • Reporting the following to the Charge Nurse and the care giving team as appropriate and/or document according to policy and procedure: o A significant change in resident’s condition, a change in a resident’s vital signs or weight, any accidents and injuries, any changes in a resident’s skin integrity, behavioral changes, signs of resident depression or anxiety, changes in a resident’s ambulation status, the level of the resident’s food consumption, and the resident’s intake of food and liquids as well as the resident’s output, when indicated. o Participate in collecting data needed for the accurate completion of the MDS and the plan of care. o Show understanding of information that is provided by the ADL flow sheets. o Provide input to the Nurses for development of the plan of care. o Provide information gathered from care giving activities to the Charge Nurse. • Deliver Resident Care: o Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current C.N.A. practices. o Perform only those activities which are within the scope of C.N.A. practice. o Follow company, facility and departmental policies and procedures, and appropriately utilize C.N.A. judgment. o Ensure accurate documentation of all medical records and reporting forms. o Promptly respond to call lights and other resident needs. o Provide oral and hygiene care pursuant to plan of care. • Customer Service: o Provide compassionate resident care services. o Establish and maintain positive interactions with internal and external customers, including coworkers and direct reports. o Take responsibility for resolving Certified Nursing Assistant issues and concerns striving for a winwin outcome. Customers include residents, families, vendors and other employees. Report issues to a nurse or other facility management when appropriate. o Present a professional appearance. • Resident’s Rights: o Know Resident Rights. Helps the residents/residents exercise and/or protect their rights. o Report resident/resident complaints to management. o Maintain confidentiality of resident/resident information. o Understand, comply with and promote all rules and regulations regarding residents’ rights. o Support resident autonomy and choice. o Promote a restraint-free environment. o Demonstrate both sensitivity and appropriate responsiveness to each resident’s “end of life” wishes. • HIPAA: o Follow and adhere to Facility’s policies and procedures implementing HIPAA requirements for the privacy and security of protected health information. o Use and/or disclose only minimum amount of Protected Health Information necessary to complete assigned tasks. o Report all suspected violation of company’s HIPAA policies or procedures to Facility Privacy Designee. • Continuing Education: o Participate in orientation, facility/department in-services and quality improvement activities. o Attend continuing education programs. o Attend facility/department/staff meetings as required. o Assist in the orientation and training of other staff. • Safety: o Know and follow all Company policies, regulations and requirements. o Demonstrate safe practices in regard to resident's comfort and safety by applying knowledge of proper body alignment for self and resident. o Practice and promote adherence to institutional safety codes. o Follow facility dress and hygiene policies. o Demonstrate proper use of equipment. Reports equipment needs or repairs. o Follow facility smoking policies. o Report and document any incidents or accidents of residents, staff or visitors to the appropriate facility personnel. o Report all hazardous conditions/equipment to Supervisor. o Perform duties, which may include transportation of residents, as assigned in Facility Disaster Plan o Use required protective equipment. o Follow infection control standards, policies and procedures. o Follow safe patient handling as required within your specific job position. o Zero tolerance for harassment and violence. o May be required to assist in the evacuation of patients during emergency situations. o Meet the general health requirements set for by the company and this facility. Rotating shifts as well as holiday and working during inclement weather may be required. Work Environment This job operates in a fast-paced nursing home environment. Also routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The responsibilities of this position involve significant physical activities including standing, lifting (up to 50 pounds unassisted), bending, stooping, pushing, pulling and twisting. All employees of skilled nursing facilities may be required to provide lifting and transfer assistance to residents. Lifting and/or transferring some residents will require use of a lifting device and /or assistance of other staff. While performing the duties of this job, the employee is regularly required to talk or hear. Ability to distinguish smells, tastes and temperatures. The employee is frequently required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. Required Education and Experience • Must be a Certified Nursing Assistant in good standing with the State, or must within four (4) months of employment successfully complete state required training and a competency evaluation program. Must maintain a current certification in the state throughout employment. • Must achieve 12 hours of in-service education annually as required by federal regulations (as well as state requirements, if exceeds federal). • Ability to adapt to a flexible work schedule. • Ability to relate positively, effectively and appropriately with residents, families, community members, volunteers and other facility staff. • Possess special interest in, and a positive attitude about, working with long-term care residents and the elderly. • Ability to read, write, speak and understand English, and be able to follow oral and written directions at a level necessary to accomplish the job. • Must be willing to assist others, including co-workers. • Must maintain regular attendance. • Meet all health requirements, as required by law. • Demonstrate basic computer knowledge with the ability to learn company documentation software. • Demonstrate understanding of State and Federal rules and regulations that govern the practices within the facility. Preferred Education and Experience • Experience in a fast-paced environment. • Post-acute care or health care experience preferred.
