Job Overview:
The Clinician provides daily case management, support and advocacy to consumers with mental health and/or substance abuse needs in a culturally sensitive and patient-centered manner.
Qualifications:
Required:
Bachelor's degree in Social Service fieldValid NJ Driver's licenseCommitment to working with clients with compassion and empathy for underserved populationsKnowledge and understanding of urban issues, hospital procedures, community resourcesStrong verbal and written communication and organizational skills to effectively work and communicate with clients, service providers, hospital staff and TRAC teamComputer literacy, punctual and reliableAble to work in stressful situations and problem solve, including verbal deescalation of consumersPreferred:
1 year of case management experienceClinical experience working with the behavioral health population, crisis intervention, and/or case management in a hospital settingBilingual in Spanish or Haitian CreoleScheduling Requirements:
Shift working, 8:30-4:30pm each dayMonday-FridayFTEssential Functions:
Attends staff meetings, workshops, trainings and supervisionConducts various assessments and screenings, not limited to: biopsychosocial, service plan, suicide risk, depression and anxietyConsults with other service providers and resources to best meet consumer needsFacilitates psychoeducational groups for consumersMaintains compliance with all organizational policies and proceduresMaintains consumers records by reviewing case notes; documenting contacts and progressManages and coordinates service (treatment) plan, making amendments as neededOther responsibilities as assigned by managementProvides on-call services for crises and emergenciesProvides pre-crisis and crisis support in person and over the telephoneProvides transportation to clients for BH and medical appointments, social services and other needs as requiredSupports, encourages and collaborates with the consumer and his/her family, providing care coordination for behavioral health, physical health needs, and social needsBenefits and Perks:
