\n\nClearly explaining assessments and obtaining patient consent\nDocumenting health concerns and implementing safety measures\nGathering and sharing key data accurately\nManaging risks when patients decline transport due to medical reasons\nIdentifying and addressing safeguarding concerns for patients and carers\nCollaborating effectively with medical teams, transport staff, and managers\nConducting patient assessments via phone or face-to-face\nAssisting staff in understanding patient needs and eligibility criteria\nOrganizing follow-up assessments, including for patients in Renal services\n\n
