The Clinical Service Coordinator is an integral member of the team, ensuring that operations run smoothly by providing administrative and logistical support to multidisciplinary clinical teams, primarily supporting registered nurses and our supplier network, to enhance clinical practice and the delivery of high-quality in-home patient care. The ideal candidate must be self- motivated, mature, and responsible with excellent critical thinking, organization, and communication skills. Strong computer skills are required. You must be able to work independently within a dynamic team environment. We are looking for creative individuals who are, first and foremost, patient centered and can thrive in a fast-paced, rapidly changing, high- profile environment.  

 

Shifts: 12 hr shifts with rotating weekends+holidays  

Overnights, West Coast: 10:30p – 11a ET (7:30p – 8a PT) What You'll Do

Principal Duties and Responsibilities:  

Communicate and build collegial relationships with clinical staff, multidisciplinary colleagues, customers and our supplier network   Review patient data for quality assurance    Maintain clinical documentation record from service providers  Simultaneously work across multiple systems while effectively managing communication response times and workload   Coordinate and schedule in-home clinical visits including MIH visits, equipment delivery, and lab tests with service provider partners    Manage and coordinate complex logistical needs of a diverse patient population   Partner with product, implementation and other internal teams to advance the MH model of care through our technology, market expansions, and developing best in class processes  Partner with leadership to onboard and train new team members   Perform other administrative support duties as required   What You Need

Competencies:  

Independent analytical and problem-solving skills  Support and collaborate with team members   Excellent organizational skills with attention to detail and ability to maintain quality of work  High level of service deliverability including multi-tasking, ability to prioritize work and meet deadlines   Effectively troubleshoot and escalate operational issues in a professional manner   Strong written and verbal communication skills, with the ability to collaborate with individuals across varying levels   Computer literacy, including command of Microsoft word, PowerPoint, and Excel with ability to learn new software as needed  Ability to demonstrate respect and professionalism for patients’ rights and individual needs, including demonstrated ability to maintain HIPAA compliance and other confidentiality  Excellent customer service skills, including the ability to manage and triage complex telephone calls  Comfortable working with a diverse patient population and multidisciplinary team, including nurses, physicians, and other clinicians as well as administrative partners including schedulers, dispatchers, etc.   Ability to uphold Medically Home culture and values while maintaining a positive and collaborative attitude   

  

Experience and Education:  

1-2 years’ professional experience required, preferably in a healthcare setting  Bachelor’s degree preferred but not required   Interest in the healthcare industry    Who We Are
