Your Role: 

Coordinate and complete timely, accurate MDS assessments 

Ensure Medicare documentation from admission to discharge 

Manage PDPM, PPS, and OBRA schedules 

Collaborate with interdisciplinary teams and MDS specialists 

Requirements: 

Active RN license (MI) 

Associate’s degree in nursing 

CPR/BLS (or complete on-site) 

MDS Certification (or willing to obtain with our support) 

2+ years as RN in skilled nursing or 5+ years in direct care (LPN/CNA) 

3+ years MDS coordination experience preferred
