Position Highlights:
The Clinical Pharmacist Specialist (CPS) positions are members of the inpatient Acute Care Clinical Pharmacy Team and focus on Internal/Hospital Medicine (IHM) within an NCI-Designated Comprehensive Cancer Center. This role will provide direct patient care via participation in interdisciplinary IHM teams managing medically complex oncology patients with acute or chronic comorbidities (e.g., infections, renal/hepatic complications, thromboembolism, endocrine/metabolic disorders, etc.). Additional responsibilities will include medication management, monitoring and optimization, clinical interventions, teaching and precepting, and participation in Department (Pharmacy and IHM) and Institutional Committees.
The CPS will have extensive clinical experience in internal medicine along with a strong background in pharmacy practice including medication safety, formulary and therapeutic guideline management, standard operating procedure and policy development, precepting pharmacy learners, and participation in clinical research or quality improvement initiatives.

The Ideal Candidate:
Doctor of Pharmacy Degree from an accredited College of Pharmacy or equivalent combination of education and experience. Has Board of Pharmacy Specialties (BPS) certification in area of specialty practice. Relevant work experience as a clinical pharmacist in the area of specialization on an interdisciplinary rounding team. Completion of a PGY-1 Pharmacy Residency and a minimum of 1-year clinical practice experience on an interdisciplinary rounding team, or a minimum of 3 years clinical practice experience on an interdisciplinary rounding team.Responsibilities:
As a part of an interdisciplinary Internal/Hospital Medicine rounding service, provide comprehensive clinical pharmacy services to assigned service’s hospitalized patients with internal medicine needs, including pharmacotherapy consultations, renal/hepatic dose adjustments, anticoagulation management, and transitions of care. Average daily census of ~15 – 20 patients. Collaborate with physicians (e.g. Internal Medicine, Infectious Diseases, Supportive Care Medicine), advanced practice providers, nurses, case managers, and other pharmacists to ensure safe, effective, and evidence-based medication use through recommendation, implementation, monitoring, and modification of individualized pharmacotherapeutic treatment plans. Develops, interprets, and provides medication related information and education for pharmacists, providers, nurses, and other healthcare personnel. Coordinates and provide medication therapy education and counseling to patients and caregivers. Support formulary initiatives, medication use evaluations, and implementation of clinical protocols or order sets related to internal medicine care in oncology patients. Precept APPE Pharmacy Students and PGY1 Pharmacy Residents as part of our ASHP-accredited Residency Program. Participate in interdisciplinary clinical committees focused on quality, safety, and guideline development.Credentials and Qualifications:
Minimum of Bachelor's degree in Pharmacy required. Licensed as a Pharmacist in the state of Florida required. Completion of a PGY-1 Residency and a minimum of 1-year clinical practice experience on aninterdisciplinary team, or a minimum of 3 years clinical practice experience on an interdisciplinary
team.
