Clinical Pharmacy Specialist (Critical Care/Transplant)
Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants can apply. Responsibilities This position is in the James J. Peters VA Medical Center in the Bronx, New York and provides support to the Pharmacy Program. The James J. Peters VAMC is a tertiary care facility classified as a Clinical Referral Level 1 Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology as well as education and research. Comprehensive health care is provided through primary care, tertiary care, and long-term care in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and extended care. James J. Peters VAMC has 311 authorized hospital beds and 120 nursing home beds and operates several regional referral points including Spinal Cord Injury (SCI) and VISN referrals for Medical/Surgical subspecialties. For example, the James J. Peters VAMC SCI Patient Care Center is the referral point for VISN's 2, 3 and 4 as well as Department of Defense (DOD) new SCI patients. The Critical Care/Transplant Clinical Pharmacy Practitioner (CPP) position is The Clinical Pharmacy Specialist functions as a therapeutic expert in drug use and drug utilization. The incumbent monitors drug use, makes recommendations on the correct utilization of drugs, exercises duties as outlined in their scope of practice, and educates medical center staff and providers on drug utilization. Clinical Functions: 1. Functions as an advanced practice provider with a scope of practice to provide services. 2. Core activities outlined within the scope of practice include: i. Executing therapeutic plans utilizing the safest and most cost effective medication treatments ii. Performing the physical measurements and objective assessments necessary to ensure the patient's appropriate clinical responses to therapy iii. Ordering, subsequent review, and action on appropriate laboratory tests and other diagnostic studies necessary to monitor, support, and modify the patient's drug therapy. iv. Ordering and prescribing medications, devices, and supplies to include: initiation, continuation, discontinuation, monitoring, and altering therapy. 3. Conducts virtual and in-person encounters with patients for patient in the Critical Care/Transplant practice area 4. Achieves minimum productivity expectations as set forth by the Pharmacy Leadership and Clinical Pharmacy Practice Office (CPPO). Program Management/Administrative 1. Collaborates in the development and maintenance of policies and procedures defining operational requirements for the Critical Care and Transplant program. 2. Completes administrative projects and reports in a timely manner. 3. Serves on VISN and/or facility level committees as requested. 4. Adheres to and ensures the Critical Care and/or Transplant program is compliant with all regulatory and accrediting body requirements, as well as facility related policies and procedures Education and Research 1. Responsible for keeping abreast of changes within local Pharmacy Service (i.e., formulary additions and deletions, interdepartmental memoranda, relevant station memoranda, policies and procedures) and maintains a general knowledge of VA, DEA and Joint Commission regulations. 2. Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other medical center and personnel as required on transplant and critical care issues. 3. Effectively instructs providers, patients and family members in the appropriate use of medications and medical devices. Can adjust communication and tracking methods based on age and developmental considerations. Is able to accurately assess and document provider and patient comprehension. Is able to refer patients to educational programs provided at the medical center by multidisciplinary teams. 4. Is skilled at using both written and verbal communication techniques in teaching others. Qualifications Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (This is required to supply if you have foreign education to meet educational requirement) Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d) Physical Requirements. See VA Directive and Handbook 5019. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Creditable Experience Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service. Grade Determinations: GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. **Note: applicants must address the KSA's in the resume to be found qualified. Duties regularly involve carrying light items such as files, computer printouts, etc. for short distances. When medication/supply orders are received, lifting up to 40 pounds, bending, stooping and stretching is required to stock shelves.
