Beth Israel Deaconess Medical Center Department of Pharmacy is seeking a Clinical Pharmacy Coordinator with advanced clinical, management and leadership training and experiences to innovate and advance a clinical pharmacy practice model for transplant services. This position reports directly to the Manager of Clinical Pharmacy Services and have frequent interactions with physicians, nurses, other members of the healthcare team, students, residents, and external professional groups. The Clinical Coordinator is responsible for the administrative and clinical direction of transplant programs and services.
**Job Description:**
1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.
2. Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.
3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.
4. Provides drug information to physicians, nurses and other health care professionals.
5. Checks and verifies sterile and non-sterile medications.
6. Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
7. Promotes a culture of patient safety by submitting medication related events and near misses.
8. Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
9. Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.
10. Oversees departmental initiatives in assigned areas of practice, serving as subject matter expert (SME) in multiple clinical specialties.
11. Coordinates the onboarding and training of new hires; Serves as the Residency Program Director as needed.
12. Supports the manager with annual employee performance evaluations within assigned practice areas.
13. Coordinates drug shortage management in assigned areas of practice through formulating mitigation strategies, providing education and execution of plan in converting medications.
14. Collaborates with the manager to oversee the development of multidisciplinary clinical guidelines and standard operating procedures.
15. Serves as the pharmacy lead on hospital interdisciplinary and/or system level committees by collaborating with clinical specialists to implement changes.
16. Collaborates with manager to create and align QI and cost initiatives in alignment with department and hospital strategic goals.
17. Oversees pharmacist staffing schedule.
18. Leads IRB approved research activities and medication use evaluations.
19. Other duties as assigned .
**Required Qualifications:**
1. PGY1 + 3-5 years OR PGY2 + 1-3 years OR 5-8 years of experience in clinical specialty area; BPS relevant to position
