Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. Works as a team member with a positive attitude when interacting with other department employees including pharmacy students and residents in addition to other hospital personnel.Displays expertise in medication therapy and exhibits the clinical and professional skills to positively interact with physicians, nurses, residents, students, and other health care practitioners.Knows and follows institutional pharmacy policies and procedures and medication policies and procedures.Knows and follows state and federal laws regarding dispensing of medications.Accurately interprets and enters medication orders in the computer for drug administration.Contacts the provider in a timely manner when there is a concern about a medication order as needed.Provides drug information, drug therapy recommendations, and education to physicians, nurses, patients, pharmacy staff, pharmacy residents and students related to assigned clinical areas.Reports adverse drug events (ADE) within assigned clinical areas.Develops department educational/monitoring tools and in-services to promote continued growth of clinical pharmacy services and up to date practice as it pertains assigned clinical areas.Develops and maintains policies and protocols as necessary related to assigned clinical areas.Participates in hospital committees related to assigned clinical areas where pharmacy involvement is needed.Reviews appropriate use of medications to include optimization of use and cost considerations.Serves as a preceptor to pharmacy residents and students, including participation in research as it applies to assigned clinical areas.Contributes to the continued development of the practice of the profession of pharmacy.Completes other duties as assigned.

 

Additional responsibilities and expectations of the Antimicrobial Stewardship Pharmacist:

Reviews patients identified through the Antimicrobial Stewardship Worklist and makes recommendations and/or interventions as indicated.Co-chairs the McLeod Antimicrobial Stewardship Collaborative Subcommittee of P&T.Collects, monitors, and reports data related to antimicrobials including, but not limited to, antimicrobial use, antimicrobial expenditures, and antimicrobial resistance trends.

Additional responsibilities and expectations of the Emergency Medicine Pharmacist:

Participates in emergency response and resuscitation including, but not limited to, cardiac arrest, trauma, stroke, STEMI, and RSI.Performs ED culture callback according to process outlined in collaborative practice agreement.Assists with outpatient prescriptions as outlined in collaborative practice agreement.

Additional responsibilities and expectations of the Floor/ICU Pharmacist:

Works primarily in a decentralized model providing comprehensive clinical pharmacy services including but not limited to pharmacokinetics, drug dosing consultations, and adult parenteral nutrition management.Participates in multidisciplinary rounds daily as applicable to assigned clinical areas.

Additional responsibilities and expectations of the Oncology Pharmacist:

Provides first check for all inpatient chemotherapy orders.Provides pharmacist cross coverage for IV therapy.Responsibilities as outlined under Floor/ICU Pharmacist

Additional responsibilities and expectations of the Opioid Stewardship Pharmacist:

Assists with policy and protocol development related to pain management and opioid use disorder.Collects, monitors, and reports data related to opioid medications including, but not limited to, opioid use, opioid-related ADEs, and pain scores.

Additional responsibilities and expectations of the Transitions of Care Pharmacists:

Assists with policy and protocol development related to transitions of care.Spearheads initiatives related to medication reconciliation and access in high-risk populations.Collects, monitors, and reports data related to care transitions including, but not limited to, ADEs, interventions, readmissions, and medication access.  

 

Qualifications:

PGY1 required.Ability to work independently required.Current, unrestricted license to practice pharmacy OR eligible for licensure to practice pharmacy in the state of South Carolina required. Licensure required within 3 months of hire.
