Purpose & Scope:
Responsible for the safe, accurate and rational distribution of pharmaceutical agents to patients of all ages. Develop and implement a plan of care utilizing disease management guidelines and standards. Provide pharmaceutical care and advanced clinical support to the nursing staff, physicians, and patients. This shall be carried out in accordance with all state and federal laws, JCAHO standards, and hospital / departmental policies and procedures.
Education:
Graduate School (College) Of Pharmacy Accredited by ACPE is required.
Doctorate in Pharmacy or Pharmacy Residency is preferred.
Experience:
Two years of experience in Healthcare/Medical - Pharmaceutical/Pharmacology or equivalent in a Hospital Clinic Setting is required.
Certification/Licensure:
Licensed as a Pharmacist by the Virginia Department of Health Professions required.
