Job Description

We are seeking a dedicated Clinical Pharmacist II to join our team, providing exceptional pharmaceutical care within a hospital setting. This role is integral to maintaining the highest standards of medication management and patient safety.

ResponsibilitiesInterpret orders and transcribe or verify them accurately and efficiently in the pharmacy computer system.Accurately compound and dispense pharmaceuticals.Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.Provide accurate and timely medication information to healthcare staff and patients.Effectively and proactively interact with medical staff regarding drug therapy issues.Document all clinical activities and interventions accurately and completely.Sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols.Supervise and assist pharmacy technicians and other support staff effectively.Train and participate in Disaster Team drills and real-event responses as assigned.Prioritize work and lead others to comply with facility medication turn-around time policy and procedures.Actively participate in staff meetings and attend staff development in pharmaceutical care.Support in the training and mentorship of new staff members as assigned.Serve as preceptor for pharmacy students and/or residents.Support and participate in the goals of the organization’s quality improvement, risk management, and patient safety program.Maintain appropriate controlled substance records.Comply with all regulatory standards.Make appropriate evidence-based, patient-centered medication recommendations.Participate in the management of medical emergencies as appropriate, including Code Blue.Provide clinical consultation and clarification to practitioners.Work with the Pharmacy Director or Operations Manager to identify and develop processes to meet or exceed all regulatory standards.Essential SkillsPharmacy, central pharmacy, hospital pharmacy experience.Pharm D. from an ACPE-accredited School of Pharmacy.Clinical pharmacy experience in a hospital setting.Completion of an accredited PGY-1 Pharmacy residency or fellowship.Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.BLS & ALS/ACLS certification.Additional Skills & QualificationsDoctor of Pharmacy from an ACPE-accredited college of pharmacy.2+ years of clinical pharmacy experience without a history of residency or fellowship.Other applicable state requirements such as sterile compounding.Work Environment

The position requires working Monday to Friday, with every other weekend from 10 AM to 6:30 PM. The work environment prioritizes advanced technology to support the team and enhance patient care, fostering a supportive, inclusive culture where employees are valued and cared for.

Pay and Benefits

The pay range for this position is $130000.00 - $150000.00/yr.

Ardent benefits- active 1st day of the month after 30 days of employment
