Responsible to provide support, education, referrals, coordination of care, and other assistance to the patient and their family prior, during, and following their healthcare experience. Responsible for facilitating integrated care through communication with strategic members of the interdisciplinary team, collaborate to provide staff education and works closely with patients, families and members of the health care team to optimize patient satisfaction and achieve optimal outcomes.
