Transitions of Care Program
Position Title: Transitions of Care Coordinator
Summary of Job Function:
The Transitions of Care Program Coordinator is responsible and accountable for assuring the delivery of quality health care services to the Transitions of Care patients. The program supports the needs of the Transitions of Care patients by design, management and implementation of the Transitions of Care Program that standardizes care and facilitates continuous improvement to achieve maximum quality outcomes throughout the continuum of care. The Coordinator works with the interdisciplinary team in the development, implementation and evaluation of data collection, analysis and reports according to established clinical practice guidelines and standards. The Program Coordinator will assist in professional education programs as requested. The Program Coordinator will provide support and resources to the Transitions of Care patient population.
