This is an 11 bed unit, you will work roughly 16 hours per week, back up for the current Clinical Liaison and the PPS coordinator, and taking the phone on the weekends. This will likely lead into a full-time position within the calendar year. Experienced is not really needed but experience in the Rehab setting will help.

Job Summary

\n

The Clinical Liaison builds relationships with current and potential referral sources. The liaison evaluates patients for post-acute care services. The liaison performs pre-admission assessments to establish if patients qualify for the inpatient rehabilitation center. The liaison collaborates with physician and nurse managers to ensure compliance with Medicare guidelines. Proper documentation is maintained both on pre-admission and throughout the patient's rehab stay.

Essential Functions of the Role

\n\nCompletes preadmission assessments timely, thoroughly, and with sufficient detail to justify a rehab admission.\nCoordinates admission process with referral source, patient/family and rehab team through effective communication and documentation.\nEducates potential patients and families, as well as referral sources on inpatient rehabilitation and the benefits of the program.\nDevelops and maintains positive business relationships with key referral sources.\nDevelops and implements strategies to grow market share in the core business line in the assigned hospital.\nPromotes BSW services and programs to the external community. Is available to assist with health fairs and educational programs as needed.\nWorks closely with service line leadership and Clinical Liaisons. Promotes rehab services by participating in educational programs, meet and greets, and interdisciplinary meetings in hospitals or other care levels.\nEnsures compliance with Medicare guidelines and proper IRF documentation both on pre-admission and throughout the patient's rehab stay.\nUnderstanding of the IRF-PAI (Inpatient Rehabilitation Facility-Patient Assessment Instrument).\nProvides exceptional customer service by ensuring patient referrals are managed and completed in a timely manner.\n\n

Key Success Factors

\n\nKnowledge of care management and resource utilization management.\nKnowledge of current Medicare PPS guidelines and regulations related to inpatient rehab compliance and IRF-PAI management.\nKnowledge and understanding of nursing and patient care standards and procedures.\nKnow laws, rules, and regulations. Understand standards and guidelines of certifying and accrediting bodies. Follow hospital and department/unit standards, protocols, policies, and procedures for nursing care in the assigned area.\nKnow medical terms, health promotion, risk reduction, and disease prevention. Understand medications, common dosages, effects, and possible adverse reactions.\nKnowledge of medical and professional nursing ethics and patient privacy rights.\nMust be able to communicate thoughts clearly, both verbally and in writing.\nSkills related to promoting and marketing a service line.\nAbility to work collaboratively with both internal and external referral sources and external partners and organizations.\nMust have critical thinking and problem-solving skills.\nGeneral computer skills include Microsoft Office, information security, scheduling and payroll systems, electronic medical documentation, and email.\n\n
