As a Dietitian on our Nutrition Team, you will work closely with the interdisciplinary healthcare team to provide medical nutrition therapy and services. Using your knowledge and education, you will develop personalized treatment plans for patients according to their medical histories, individual needs and goals. This role is integral to our comprehensive model of care, encompassing every aspect of wellness and health. 

 

A caregiver in this position works days from 7:30AM – 4:00PM. This position will float to Medina, Lodi, and Akron Hospital.  

 

A caregiver who excels in this role will:  

Develop and implement an individualized plan for nutrition intervention in accordance with the patient's medical program goals and objectives, nutrition prescription and self-management training.  

Evaluate the effectiveness and measures progress toward outcomes of medical nutrition therapy interventions utilizing assessment and evaluation techniques that consider the varied needs of specific populations. 

Provide appropriate documentation that summarizes the nutrition care plan in the patient's medical record, including nutrition assessment, diagnosis, plan, implementation and progress toward goals. 

Communicate with the medical team through interdisciplinary meetings, rounds and medical conferences. 

In cooperation with the food service staff, assure conformance to diet prescriptions. 

Participate in nutrition research studies and communicate findings through reports, abstracts, presentations and publication. 

Provide nutrition counseling for individuals and groups and evaluate achievement of learning objectives by the patient and/or family. 

Provide appropriate follow up in accordance with treatment goals, referring patients for outpatient counseling when needed. 

Serve as a resource to medical staff and allied health personnel on nutrition issues.  

Conduct in-services and educational presentations to hospital/department staff and assist in the education of dietetic students and interns.  

Maintain accurate record keeping of daily clinical activities, including information for scheduling and billing. 

Assist in developing and revising patient education materials, clinical forms, protocols, nutrition-related policies, procedures and goals. 

Participate in departmental and interdisciplinary meetings, task forces, projects and community projects/education as needed/assigned. 

 

Minimum qualifications for the ideal future caregiver include:  

Bachelors Degree in Dietetics, Foods and Nutrition or a related area 

Completion of a Didactic Program in Dietetics and Supervised Practice Program approved by the Commission on Accreditation for Dietetics Education (CADE) of the American Dietetic Association 

Registered Dietitian or eligible for registration by the Commission on Dietetic Registration (CADE) of the American Dietetic Association 

Comprehensive knowledge and application of nutrition services and Medical Nutrition Therapy
