Full Time Days **This roll is mostly remote with travel to the Munson Primary Care practices across the region** This is the Lead Care Manager position responsible for training and supporting Nurse Care Managers, Nurse lead Annual Wellness Visits and other clinical programs under the physician hospital organization The CIN Clinical Coordinator is responsible for support and training of Ambulatory Care Coordinators (CM) across the system and for Independent Practices that are part of the PHO. Working side by side with CM’s to improve workflows and efficacy and holding training classes for new CM’s. As, CIN Clinical Coordinator there is also system responsibility for the Michigan Collaborative for Type 2 Diabetes (MCT2D). MCT2D is a quality collaborative partnering with U of M and BCBSM to improve DM care. This brings provider and CM activates together, his includes partnership with local, regional and statewide groups leading care management and improving diabetic outcomes.
