**Clinical Case Manager**
**Position Summary**
Under the general supervision of the program supervisor or designee, the Clinical Case Manager works to make a meaningful difference in the lives of individuals. This position works closely with community partners, providing advocacy, professional services and support towards the goal of helping the individual live successfully in the community.
**Responsibilities** :
+ Performs all work in a manner consistent with NYAP’s mission, values and philosophies.
+ Provides case management, natural support/kin linkage, linkage to community resources, advocacy, and other professional supports on behalf of the individual served.
+ Applies evidence-based services modality in service delivery and documentation.
+ Screens referrals for mental health services, determine severity and services needed.
+ Works as a member for the care team to meet the needs of the customer and to support program goals.
+ Assists clients in obtaining public entitlement benefits and proper documentation needed.
+ Assists clients in scheduling medical appointments and obtaining medical care.
+ Assists clients in medication management and medication compliance.
+ Applies clinical skills to help screen individuals for the program.
+ Provides case consultation and clinical support to the treatment team.
+ Fosters people skills to be able to connect with clients.
+ Develops comprehensive front-end assessment of potential clients.
+ Completes clinical assessment of potential clients.
+ Drives/travels to provide community-based services as needed.
+ Participates in weekly clinical staffing meetings to provide clinical feedback to assist clients in meeting their treatment goals.
+ Utilizes standardized assessment/screening tools effectively in order to help gage client appropriateness for services.
+ Diagnoses mental illness with knowledge of DSM VI.
+ Creates linkage and referrals for follow up services.
+ Provides individual therapy.
+ Assists clients in the community with adult living tasks such as grocery shopping, bill payments, and other household tasks.
+ Functions fluently in usage of electronic health record system.
+ Assists clients in their home with maintaining the home environment and adult living skills.
+ Actively participates in continuous quality assurance processes, weekly supervision/program, and planning meetings.
+ Performs other duties as assigned by the supervisor.
**Minimum Qualifications**
+ Bachelor’s Degree in Social Work (Master’s Degree preferred) or comparable Human Services field from an accredited institution.
+ 3 years of work experience working in direct service with individuals and families strongly preferred.
+ Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
+ A willingness to work flexible and non-traditional hours in the service of the individuals and persons served.
+ Capacity to be flexible and responsive to system partners and internal customers.
+ Capacity to remain objective and professional in all areas of job function.
+ The ability to demonstrate tolerance and respect for the ideas and actions of others.
+ The capacity to effectively work with and to be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.
+ Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs.
**Driving and Vehicle Requirements**
+ Valid driver’s license
+ Reliable personal transportation
+ Good driving record
+ **Minimum automobile insurance coverage of $100,00/$300,000 bodily injury liability**
