The Clinical Social Worker provides comprehensive psychosocial support to high-risk patients and their families with complex needs (i.e., traumas, mental health crises, substance use, protective services, grief etc.). This role involves conducting clinical assessments, developing, and implementing care plans, offering therapeutic intervention, and facilitating smooth transitions of care. Provides a wide array of advanced clinical social work services to patients and families to promote understanding and resolution of problems related to environmental stress, physical illness or injury, interpersonal conflicts, and other psychosocial issues with a primary focus on wellbeing, safety, and mental health. They may self-refer or receive referrals for patients from interdisciplinary team members and are responsible for collaborating with the internal and external care team(s).
