ResponsibilitiesA day in the life of a Clinical Advocate at Hackensack Meridian Health includes:
Assess learning needs of individuals with diabetes, develop the plan of care, implement appropriate interventions and evaluate the effectiveness of teaching using teach back.Interpret and implement the philosophy and objectives of The Inpatient Diabetes Program to members of the team, medical staff and the other appropriate departments.Assure policy, as it relates to diabetes, is being followed so as to provide best practice in the care of the diabetes population.Specialize in the application of educational theory required for the treatment and management of diabetes.Collaborate with other disciplines to provide a team management approach to best practice in the care of the diabetes population.Participate in multidisciplinary rounds, diabetes team meetings, and other hospital committees related to diabetes.Act as a resource to hospital staff for diabetes related information and materials.Efficient in maintaining the requirements necessary for the Joint Commission Certification for Disease Specific Care- Diabetes.Implement and deliver a diabetes education course for nursing staffParticipate in research projects/programs as appropriate.Utilize and participate in current research findings in the care of patients and families following standards of care, policies and procedures and protocols.Adhere to the standards identified in the Medical Center's Organizational Competencies Qualifications Current and valid New Jersey Nursing license is requiredBSN required; MSN preferredBLS/CPR certification by American Heart Association is required Minimum of two (2) or more years of experience in nursingKnowledge of case management required National Certification as a CDCES is preferred, must be in progress/obtained within 6 months of hireIf you feel that the above description speaks directly to your strengths and capabilities, then please apply today!
Compensation
Starting at $106,329.60 Annually
HMH is committed to pay equity and transparency for our team members. The posted rate of pay in this job posting is a reasonable good faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market-competitive total rewards package.

Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
