Overview Clinic Nurse Practioner Rogue Valley Manor is proud to be Southern Oregon’s premier not-for-profit Life Plan Community. The Manor is a thriving, bustling village of nearly 1,000 incredible residents, with a sprawling, magnificent 668-acre campus that’s full of active, engaging ways for residents to spend their time. Our campus offers residents delicious dining and a continuum of healthcare including an on-site ambulatory clinic, Assisted Living, Memory Care and Skilled Nursing. Our internal medicine clinic is a patient-centered clinic focused on providing comprehensive, compassionate, and evidence-based care to adult patients. We treat a wide range of chronic and acute medical conditions, with an emphasis on preventive health and long-term wellness. Our team is committed to building lasting relationships with patients while maintaining a supportive and respectful work environment. You will provide direct and indirect care for residents according to the individualized needs of the patient, in accordance with the state Nurse Practice Act. You will work in conjunction with the physicians and clinical staff to ensure established standards and practices are followed. Understand the importance of resident rights in healthcare as well as the community as a whole, and advocate for operations, processes, and decisions that are in residents’ best interests. Conduct comprehensive health assessments, including physical exams, medical history review, and diagnostic tests. Develop individualized care plans, prescribe medications, and implement treatments in accordance with state laws and regulations. Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to deliver high-quality, coordinated care. Monitor patient progress, adjust care plans as necessary, and provide education to residents and families on health management and wellness. Perform routine follow-up visits, manage chronic conditions, and address acute health issues in a timely manner. Lead wellness programs, preventive care initiatives, and health education workshops tailored to the needs of residents. Document all patient interactions and care provided in electronic health records (EHR) in compliance with facility policies and state regulations. Participate in interdisciplinary team meetings, quality improvement initiatives, and ongoing training to stay current with best practices and clinical guidelines. Provide support in end-of-life care planning and palliative care, ensuring residents' comfort and dignity. Serve as a mentor and resource for nursing staff and assist in developing care protocols and clinical guidelines. Support and contribute to effective safety and risk management efforts by adhering to established policies and procedures, maintaining a safe environment, promoting accident prevention, and identifying and reporting potential liabilities. Use protective equipment and takes appropriate precautions whenever there is potential for contact with blood or body fluids. Provide care appropriate to the age of the patients. Obtain appropriate medical history, physical exam on patients, and record data. Interpret data to determine appropriate diagnostic and therapeutic procedures as needed. Collaborate with physicians as needed in managing acute and long-term medical needs of patients. Prescribe and administer medications using the Nurse Practitioner drug formulary. Assist in the integration of data systems, clinical protocols, and utilization review into community healthcare operations, as appropriate. Work with leadership to develop department and personal performance goals, objectives and standards. Perform other duties as assigned. About you: Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution. Valid Nurse Practitioner (NP) License from the state Board of Nursing. National certification as a NP (Family, Adult-Gerontology, or other applicable specialty) through ANCC or AANP. Minimum of 1 year of experience providing patient care. Proven ability to establish therapeutic relationships with diverse age groups and medical needs, demonstrating patience, care, understanding, and effective problem-solving skills. Proficiency with computer systems and electronic health records. Working knowledge of regulations regarding operation of a healthcare clinic, HIPAA, Medicare/Medicaid and OSHA safety regulations. Effective organizational skills to manage schedules, coordinate services, and ensure that all necessary documentation and paperwork are completed accurately and on time. Strong understanding of geriatric care principles, chronic disease management, and preventive healthcare. Ability to work independently and as part of an interdisciplinary team in a fast-paced environment.
