Position Summary:

The primary purpose of the chaplain is to work in consultation with the interdisciplinary team to assess the spiritual needs of patients and families and to provide spiritual counseling to meet these needs in accordance with the patient’s and family’s acceptance of this service, and in a manner consistent with patient and family beliefs and desires.  The chaplain /Spiritual Counselor makes all reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient’s spiritual needs.

 

Department Summary:

The spiritual care department is available 24 hours a day to provide spiritual and emotional support to patients, families, visitors, and our employees.

 

Qualifications:

4 units of CPE credits preferred  High school graduate and bachelor’s degree preferred One-year pastoral care or counseling experience in healthcare setting preferred Excellent interpersonal and oral/written communication skills Current valid driver’s license in good standing Basic computer skills Must have own transportation

Responsibilities & Expectations:

Daily rounds and answering referrals from patients, families, and staff

 

Oversee/promote daily operation of spiritual care Oversee and coordinate chaplain on call schedule as well as Catholic Priest on call schedule Assist and oversee Eucharistic ministers’ program at Aultman Hospital, Woodlawn, and the Compassionate Care Center Participates in various committees such as Ethic or end of life Assesses the spiritual status of the patient, family, and caregiver Patient documentation in the appropriate electronic medical record Participate in Strength for the Journey for employees Assist with funerals, weddings, baptisms, protestant communion and other sacraments as requested from patients or families   Assist with annual employee memorial service for employees, physician, veterans Host special occasion prayer services along with praying for prayer requests from the prayer boxes Abides by the Aultman Code of Conduct Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health System Completes annual competency requirements Consistently communicates requests, suggestions, concerns or problems to the appropriate personnel Communicates with and supports the involvement of local clergy and/or spiritual counselors as possible and as desired by the patient, family, and caregiver Participates with the interdisciplinary team in the development and implementation of the patient’s and family’s plan of care Consults with and provides education to the interdisciplinary team members and patients and families about spirituality and related care and services Participates in volunteer eucharistic minister training, bereavement services, annual memorial service and community education as requested Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community Consistently completes assigned responsibilities in required time frame and accepts additional duties and responsibilities as requested by Unit Director Protects patient health information according to HIPPA Privacy Rule and related Aultman policies Complies with agency infection control and safety policies Demonstrates flexibility in meeting the needs of the program and responds positively to changes    in workflow Communicates with and supports the involvement of local clergy and/or spiritual counselors as possible and as desired by the patient, family and caregiver Daily rounds and answering referrals from patients, families, and staff

Hospice and Palliative Care

Acknowledges and respects the patient’s and family’s beliefs, culture and values related to    life’s meaning including suffering and loss and desire for service/support Documents the spiritual assessment, goals for spiritual care, services provided and the patient’s     and family’s response to spiritual care Interventions may include meditation, counseling, prayer, sacred rituals or practices, active listening, and supportive presence Communicates with and supports the involvement of local clergy and/or spiritual counselors as possible and as desired by the patient, family, and caregiver Participates with the interdisciplinary team in the development and implementation of the patient’s and family’s plan of care Consults with and provides education to the interdisciplinary team members and patients and families about spirituality and related care and services Participates in volunteer eucharistic minister training, bereavement services, annual memorial service and community education as requested Displays a positive and professional image and attitude in all relationships with patients, families, peers and in the community Participates in the Quality Assessment Performance Improvement (QAPI) program

Working Conditions:

Work 8-10 hours per day depending on location. This unit requires 24/7 coverage. Work weekends and holidays as assigned per on call schedule Cross training in all areas in the department is required. On call coverage is assigned to cover the department schedule Staff may be assigned to different locations within the foundation Travel within a 40-mile radius is required
