Summary

Position Highlights:

Assesses, plans and implements specific treatment programs for individual patients according to the standards of nutrition therapy. Provide care for inpatient and/or outpatient caseload. Occasional weekend and/or holiday coverage staffing needs warrant. Collaborate with an interdisciplinary treatment team regarding the needs of the patient. Contributes to presentations, publications, supervise students/interns, and execution of departmental education efforts

Responsibilities:

Complete initial assessments of assigned patients in accordance to established standards of care and practice including selecting assessment tools, tests and measures, goal setting, etc. Monitors and responds to the patient's response to intervention and adjusts accordingly, reassessing the patient as needed or a minimum follow-up per department standard. Attend interdisciplinary discharge rounds, communicate with the interdisciplinary team regarding patient status and discharge planning. Attends monthly staff meetings and educational in-services/presentations. Present in-service after attending professional conference Accurately performs documentation on each consult and follow-up within the electronic medical record and accurately documents billing charges within Medicare guidelines. Completes charting in medical record upon completion of session with no more than 2 billing errors per month Adheres to all departmental expectations and deadlines as outlined in the Certified diabetes educator expectations which are provided during the first week of employment. Covers occasional weekends and holidays

Credentials and Qualifications:

RN - 3 years of clinical RN experience in an acute care hospital. 1 year experience as Clinical Diabetes Educator. Or RD - 3 years of clinical RD experience in an acute care hospital. 1 year experience as Clinical Diabetes Educator BLS Certification Registered Dietitian (RD) or Registered Nurse (RN) Certified Diabetes Educator (CDE) Bachelor’s Degree in Nursing or Nutrition
