\nThe Certified Diabetes Educator works with people at risk and with diabetes to achieve behavior change goals which lead to better clinical outcomes and improved health status.
\nESSENTIAL FUNCTIONS OF THE ROLE
\n\nPlans, directs, and provides individualized self-management education, training and help to patients and families in a culturally-competent manner across the lifespan.\nDevelops and conducts educational programs and in-service training programs.\nServes as a resource to staff and community as a diabetes care and education specialist.\nOptimize care delivery, patient and provider experience, quality, safety, and costs.\nApplies current principles of teaching and learning and/or behavior change to facilitate self-management skills.\nTailors interventions to individual self-management needs.\nCommunicates information to patients and healthcare team in a timely manner. Serves as a role model, resource, and mentor to other team members.\nParticipates in quality improvement activities to help process improvement and clinic/facility/system goals.\nParticipates in professional development activities to maintain and improve required qualifications; obtain 15 diabetes-related Continuing Education hours annually.\n\nKEY SUCCESS FACTORS
\n\nAdvanced knowledge of pre-diabetes and diabetes care modalities, therapies, terminology and equipment.\nDemonstrates familiarity with pre-diabetes and diabetes pathophysiology, epidemiology, and evidence-based clinical practice.\nDemonstrates familiarity with ADA Education Recognition requirements or National Standards for Diabetes Self-Management Education and Support, if in an ADA Recognized program.\nMust be able to communicate thoughts clearly; both verbally and in writing.\nAbility to effectively use a variety of teaching methods.\nKnowledge of the geographic area, the community and its resources.\nCultural proficiency related to health practices.\nCommitted, dependable, adjustable, original, resourceful and has an excellent desire to help the community.\nAble to interact positively with patients as needed.\nGeneral computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, hand held scanning and email.\n\nBelonging Statement

QUALIFICATIONS

Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.Registered Dietitians (RD), Registered Nurse (RN), Pharmacist (RPH): Must have one of the following:RD - Registered DieticianRN - Registered NurseRPH - Pharmacist.
