and Geriatric Services Job Summary Working on a multidisciplinary team including a clinician and board certified Child and Adolescent Psychiatrist co facilitating groups providing therapy to children and adolescents aged years old Participation in weekly meetings to discuss patient progress in treatment and their needs collaboration with community providers schools DCF workers etc to ensure continuity of care Support the completion of intakes referrals and discharges Provide transport in company vehicles to clients living in the catchment area upon obtaining Public service endorsement
