Case Manager (Workforce & Education)
525 Washington Street, Buffalo, NY, USA | Workforce Development - Case Managers | Hourly | 23.41-25.85 per hour Depending on Experience | Full Time | Health, Dental, Vision, PTO, 403B, Paid Holidays Overview Catholic Charities of Buffalo is seeking a full-time Case Manager for our Workforce and Education department located at 525 Washington Street, Buffalo, NY 14203 . Responsibilities will include working with participants who are in need of academic functioning level upgrades, High School Equivalency preparation, assistance with career and employment, and/or entrance into college, vocational training programs, or military. Will conduct initial in take and comprehensive assessments. Determine eligibility for enrollment into funded programs. Will act as a conduit for the flow of information related to referrals from community organizations, agencies, courts, probation, and school districts for the purpose of establishing and maintaining an effective network specific to case management services. Act as a positive team member and develop working relationships with peers and community partners. Job Responsibilities + Provide case management services to participants + Develop working knowledge and relationships with school districts, government, and not-for-profit sectors + Learn and utilize program specific data tracking system (OSOS) + Determine eligibility for enrollment into funded programs + Become familiar with the completion of paperwork necessary for entrance into training programs or college that includes admissions applications, FASFA forms, PELL grants and scholarships + Respond to consultation requests from educators, employment counselors, participants, and families + Perform follow-up services suggested and track to ensure quality of care as specified by program contracts + Become familiar and comfortable with providing crisis intervention, assessment, and referral to participants on scheduled or walk-in basis + Develop and become more comfortable with conducting interviews, counseling and/or mentoring to assist individuals with gaining insight into causes of personal needs and promotion of personal development + Participates in the team approach with the education and employment components + Develop other resources as needed + Meet quantitative expectations established by the funding sources and agency + Engage in professional development and in-service trainings + Perform other duties as assigned + Comply with the Agency's Compliance Program, Code of Ethics, laws, regulations, and ethical standards applicable to your job duties. Minimum Qualifications + Bachelor's degree in social work, school or career counseling, guidance, education, or related human services field. + 1 to 2 years of experience within social work, school counseling, guidance, or related human services preferred. + Strong personal and interpersonal communication skills. + Excellent organizational skills. + The ability to keep concise records. + A sensitivity to the development stages and well-being of participants. + The ability to work with a diversity of individuals and groups. + Able to accept supervision and work well both independently and in teams. + Excellent problem-solving skills to analyze issues and create action plans. + The ability to adapt to changing work priorities and maintain flexible working hours. + The ability to establish and maintain constructive relationships. + Must have reliable transportation and a valid NYS driver's license for required travel.
