Case Manager

Current Licensure in Social Work or in a clinical or related discipline (RN preferred), per state guidelines for case management and/or discharge planning, required

Responsibilities

 

Responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomesCoordination of care across the continuumPromotion of cost-effective care within the allotted time frameAssuring payments of hospital-based services meeting patient-related utilization management criteriaImplementation of safe and appropriate discharge plans.  Assesses the social work needs of the patient and provides social work intervention as part of the discharge planning process.  Clinical Interventions/Discharge and Care Planning ManagementFiscal Management;Payer/Referral Management.  Qualifications

Minimum Qualifications

Current Licensure in Social Work or in a clinical or related discipline (RN preferred), per state guidelines for case management and/or discharge planning, required. 

Previous Experience 

Previous experience in Case Management and Discharge Planning preferred. CCM Certification Preferred.
