Job Title: Case Manager RN

Job Summary:

One of our large healthcare clients is seeking a compassionate and experienced Registered Nurse Case Manager to oversee clinical patient caseloads and ensure the delivery of high-quality, coordinated care. In this role, you will collaborate with interdisciplinary teams to develop and implement effective care plans, guiding patients and providers toward optimal health outcomes.

Key Responsibilities:

Manage and monitor clinical caseloads within the hospital setting.Develop, implement, and evaluate individualized care plans.Collaborate with physicians, nurses, social workers, and other care team members.Educate and advise patients and providers on care coordination and best practices.Ensure compliance with managed care guidelines and hospital protocols.

Required Qualifications:

Active Registered Nurse (RN) license.Minimum 1 year of case management experience in a hospital setting.Strong understanding of managed care principles and care coordination.Certification in Case Management, Nursing or Utilization Review is preferred.Experience working with interdisciplinary teams in acute care environments.

Work Environment

This position is based in a hospital office setting, with access to clinical resources and support systems necessary for effective case management.

Pay and Benefits

The pay range for this position is $80000.00 - $90000.00/yr.
