Job Title: Case Manager RNJob Description We are seeking a dedicated Case Manager RN to manage the hospital clinical patient caseload and ensure the implementation of proper care plans for optimal patient care. You will collaborate with various departments and partners to execute care plans and provide direction and advice to patients and providers on effective care management. Responsibilities + Manage hospital clinical patient caseload. + Ensure proper care plans are implemented for patient care. + Collaborate with various departments and partners to implement care plans. + Direct and advise patients and providers on proper care management. Essential Skills + Registered Nurse certification. + Experience in case management within a hospital setting. + Strong skills in managed care. Additional Skills & Qualifications + Certified Case Manager (CCM) certification. + Minimum of 1 year of case management experience in a hospital setting. Work Environment This role is based in a hospital office environment, where you will have access to the necessary resources and facilities to perform your duties effectively. Pay and Benefits The pay range for this position is $80000.00 - $90000.00/yr.
