Day-To-Day:

Completes needs assessments regarding family reunification needs.  Monitor clients progress on adherence to individualized treatment plans and to develop and implement treatment plan Provides active, intensive, case management services with focus on family reunification, including accompanying the client to the referral source if necessary.  Interacts with Texas Department of Family and Protective Services (CPS) and provides advocacy for reunification  Works collaboratively with schools and community mentoring youth mentoring organizations  Conducts mother and family meetings

Qualifications

Bachelor's Degree  in a social, human services, criminal justice, behavioral science program or related field from an accredited college or university and 1 year experience.  Must have valid Texas Driver License, clear MVR and ability to be insured.
