Description
A Case Manager plays a vital role in coordinating and providing comprehensive support to clients by developing individualized care plans specific to their needs. This professional serves as an advocate and liaison between clients, service providers, healthcare professionals, and community resources to ensure that clients achieve their goals effectively and efficiently.
Key Responsibilities:
Client Assessment:
+ Conduct thorough assessments to evaluate clients’ needs, challenges, and goals.
+ Review documentation, interview clients, and coordinate with other professionals to develop a complete understanding of each case.
Care Planning:
+ Create and implement personalized case management plans in alignment with the client’s objectives.
+ Periodically review and adjust plans based on clients’ progress, changing circumstances, or feedback.
Coordination and Advocacy:
+ Act as the primary point of contact for clients and coordinate services with internal teams, healthcare providers, social service agencies, and other relevant parties.
+ Advocate for clients to ensure they have access to appropriate community resources, care services, and financial support when applicable.
Monitoring and Support:
+ Monitor the execution of case plans to ensure all goals are being effectively addressed.
+ Provide ongoing support and guidance to clients in navigating complex systems or challenges.
Documentation and Reporting:
+ Maintain comprehensive and accurate records for all assigned cases in compliance with organizational policies and legal standards.
+ Prepare regular case reports and provide updates to supervisors and stakeholders as required.
Professional Development:
+ Keep current with industry best practices, regulatory requirements, and community resources through training programs.
+ Participate in team meetings and share lessons learned to improve organizational practices.
Requirements
Education:
+ Bachelor’s degree in Social Work, Psychology, Nursing, Healthcare Administration, or a related field (required). A master’s degree is preferred.
Experience:
+ 2+ years of case management experience in a healthcare, social services, or professional services environment. Hands-on experience with complex cases and multidisciplinary teams is an advantage.
Skills:
+ Strong organizational, communication, and problem-solving skills.
+ Ability to build lasting professional relationships with a diverse range of stakeholders.
+ Familiarity with case management tools or software.
Certifications:
+ Relevant certification such as Certified Case Manager (CCM) or Social Work licensure is a plus.
