Position Purpose
At Mercy Care, we are committed to serving Atlanta’s most vulnerable populations with compassion and dignity. As a Case Manager, you will play a vital role in helping individuals and families navigate complex challenges by providing client-centered support and connecting them to essential services. This position is ideal for someone passionate about social justice, advocacy, and holistic care coordination.
What You Will Do
As a Case Manager, you will:
Conduct comprehensive assessments to identify client needs, strengths, and barriers.
Develop individualized service plans with achievable short- and long-term goals.
Connect clients to housing, healthcare, employment, behavioral health, and other community resources.
Monitor progress through regular follow-ups and adjust care plans as needed.
Maintain accurate documentation in compliance with HIPAA and organizational standards.
Collaborate with internal teams and external partners to ensure continuity of care.
Advocate for clients to access and retain benefits.
Facilitate crisis interventions, discharge planning, and case conferencing.
Participate in team meetings, supervision, and ongoing professional development.
Minimum Qualifications
Education:
Required: Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or related field.
Preferred: Master’s degree in a related field.
Certifications:
Required: CPR/First Aid
Preferred: Certified Case Manager (CCM) or equivalent.
Experience:
Required: At least 1 year in case management or social services.
Preferred: 2–3 years working with underserved populations such as the homeless, elderly, or those with chronic health conditions.
