Job Responsibilities Conduct intake and assessments to create timely individual service plans with measurable goals and objectives, provide short and long-term case management services to individuals and families. Knowledgeable about community resources and advocate for services with community partners. Works collaboratively with therapists to support clients and have a flexible schedule to meet the long-term and short-goal needs of the clients, which may include evening or weekend support. Maintain up-to-date records, files, databases, and routine reports. Must meet all required productivity standards. Solicit, log, sort, store any donations or deliveries for the program, and engage in community outreach and driving through mobile van efforts. Adhere to agency and programmatic policies and procedures, professional standards of conduct, and any relevant licensing and accrediting bodies for performing job expectations. Participate in community meetings and represent the agency regarding services and timely reporting of program needs.
