Case Manager
Catholic Charities of the Archdiocese of Chicago partners with mission-driven people and organizations across Cook and Lake counties to witness a message of mercy and hope to a world in need by improving lives, nourishing spirits; strengthening and empowering people, families, and communities; and committing to being present to our times through the eyes of faith.   Case Manager Provide case management and community outreach, develop service plans, and collaborate with partners to support client goals while maintaining records and program standards. Job Responsibilities Conduct intake and assessments to create timely individual service plans with measurable goals and objectives, provide short and long-term case management services to individuals and families. Knowledgeable about community resources and advocate for services with community partners. Works collaboratively with therapists to support clients and have a flexible schedule to meet the long-term and short-goal needs of the clients, which may include evening or weekend support. Maintain up-to-date records, files, databases, and routine reports. Must meet all required productivity standards. Solicit, log, sort, store any donations or deliveries for the program, and engage in community outreach and driving through mobile van efforts. Adhere to agency and programmatic policies and procedures, professional standards of conduct, and any relevant licensing and accrediting bodies for performing job expectations. Participate in community meetings and represent the agency regarding services and timely reporting of program needs. PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned. X Kneel and move from sitting, bending, kneeling or standing multiple times a day. X Push and pull objects up to 25 pounds X Climb up and down up to 3 flights of stairs at a time. X Lift up to 25 pounds. Knowledgeable in computer and technology skills. Ability to drive mobile van with support staff Minimum: 1-2 years of relevant experience Computer literacy and data entry knowledge required Be able to advocate and coordinate resources for individuals within underserved communities.
