On With Life’s Community Services team is adding a Case Manager! Our Case Managers oversee critical processes related to the care and treatment of persons served, including:
+ Managing referral information, communication, and collaboration with referral sources.
+ Assisting with admission processes for both location including but not limited to admission packets and assessments.
+ Facilitating discharge planning and transition of care for persons served.
+ Advocating and communicating persons served needs to community-based providers, case managers, and other relevant parties.
+ Participating in special projects and other duties as assigned based on role and responsibilities.

This full-time position primarily works Monday through Friday, 8am - 4:30pm.

An excellent candidate for this position meets the following criteria:
+ Proficiency with Microsoft Office products (Office 365 preferred).
+ Ability to manage confidential information, multiple priorities, and multiple tasks while maintaining attention to detail.
+ Strong ability and commitment to accuracy and collaboration.
+ Strong written and verbal communication skills.
+ Independent decision making and problem-solving abilities.
Preferred experience:
+ Experience working with and/or managing referrals or person served caseloads.
+ Experience with Point Click Care.
+ A bachelor's degree in a related or applicable field.
