Description We are seeking a highly organized and compassionate Case Manager to join the team at a local non-profit on a contract basis in Los Angeles, California. In this role, you will support clients by connecting them with vital community resources, managing their cases, and ensuring accurate and timely documentation. Key Responsibilities: + Conduct comprehensive intake assessments to develop individualized service plans. + Provide ongoing case management and offer crisis intervention support when needed. + Connect clients with community resources, including housing, healthcare, employment, and mental health services. + Maintain accurate and up-to-date case documentation in compliance with program and funding requirements. + Collaborate with internal team members and external service providers to coordinate high-quality care. + Participate in regular team meetings and training opportunities to enhance knowledge and performance. Requirements • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field, or equivalent work experience. • Previous experience in case management or working with vulnerable populations is preferred. • Strong organizational skills and the ability to manage time effectively in a dynamic environment. • Demonstrated compassion, patience, and adaptability under pressure. • Proficiency with tools such as ADP, CRM systems, Concur, and Cisco Webex Meetings. • Bilingual or multilingual abilities are a plus.
