Case Manager
Description We are looking for a dedicated Case Manager to join our team in Fresno, California. This Contract-to-Permanent position offers an opportunity to make a meaningful impact by assisting families with essential services and ensuring compliance with organizational and funding guidelines. The ideal candidate will bring strong case management expertise and a proactive approach to supporting clients and providers. Responsibilities: • Provide timely and responsive support to families, addressing their needs and concerns effectively. • Meet with walk-in clients and new providers to review necessary documentation and ensure compliance. • Process and manage client correspondence, including change of address forms and return mail procedures. • Handle cases requiring compliance actions by issuing appropriate notices or processing terminations when required. • Maintain accurate records and perform eligibility verifications to ensure compliance with agency policies. • Address parents' concerns with empathy and provide referrals to additional resources as needed. • Collaborate with administrative staff to perform duties aligned with agency policies and funding regulations. • Manage case files using case management software to ensure seamless tracking and reporting. • Perform additional responsibilities as assigned by administrative staff to support organizational goals. Requirements • Minimum of 2 years of experience in case management or a related field. • Proficiency in case management software and tools. • Strong knowledge of eligibility verification processes, including Medicaid and patient eligibility. • Ability to address client concerns empathetically and provide appropriate referrals. • Familiarity with agency guidelines and funding regulations. • Excellent organizational and communication skills. • Attention to detail in managing documentation and compliance procedures. • Certificate Authority experience is a plus.
