We are recruiting for a Case Manager that is dedicated and detail-oriented to support individuals and families seeking rental assistance and housing stability. This role involves conducting initial client assessments, developing individualized housing plans, and ensuring successful linkage to long-term housing and mainstream benefits. The Case Manager will be responsible for documenting services, maintaining accurate data in HMIS and EmpowOr systems, and preparing reports in compliance with funding requirements. Additional responsibilities include preparing requisitions, coordinating family-focused events, and providing up to three months of follow-up assistance after discharge. The ideal candidate will be organized, compassionate, and committed to supporting vulnerable populations on their path to permanent housing.
Education: Bachelor Degree in Sociology or related discipline; or Associate Degree with case management experience.
Bilingual: English/Spanish
Valid NJ Driver’s License
Must be fully vaccinated against COVID-19
