Responsibilities

Position Description: Reports Directly to Regional Administrative Manager for Foster Care Services. Functions as an active, co-participant with the Case Planning Team in providing services to Children and Families. The Case Aide supports the Case Planning team to promote and expediate permanency for our children and families. 


Duties Included but not limited to:

Job Responsibilities

Work as part of an interdisciplinary team to support and expediate the service delivery to our youth and families.Provide support services to Families including advocacy, information sharing and resource referrals such as parenting skills, community MH services, HRA assistance, escorting to appointments, etc.Conduct workshops for parents with focus on training them on the completion of housing applications, food stamp and health benefits applications, and other applications for services.Responsible to assist Case planning team with driving agency vehicles to transport clients (children and families) to hospitals, residential settings, medical appointments, shopping emergency admissions and emergency replacement of clients etc.Assist Case Planning Team with the delivery of concrete services and supervising visits where appropriate.Responsible for submitting Visitation Notes, Meeting Minutes and all necessary paperwork in a Timely Manner.Responsible for attending Case Conferences, PPC, Treatment Team Meetings and Trainings as needed to support the Case planning team.Research and develop a regional resource guide of community programs/resources to link families up for services post discharge from the agency.Regularly collaborate and communicate with staff by responding to emails, maintaining an up-to-date outlook calendar, participating in supervision meetings, and scheduling office days.Complete all Trainings necessary to fulfil this role. The trainings include Family Time Policy, Emergency Guidance on Family Time-COVID, assessing from Moving to Supervised Visits, Maximizing Family Time, Visitation Policy, NYF Foster Care Family Time CNNX, Evolv- Data Entry and Other Trainings as neededOther duties or special projects as assigned.

Qualifications

Requirements:

High School Diploma or GED equivalent requiredValid Driver's License required.

Prior experience and demonstrated skills working with children and families; Listening, engagement, and basic supportive counseling skills; ability to work as part of a team; good organizational and writing skills.

bilingual (Eng/Span) strongly preferred.
