Care Transitions Assistant
Serves as a liaison between Medicare patients and the hospital for distribution of the Important Message From Medicare and the Code 44 letter. Interacts with patients, families/patient representatives to assure understanding of the Medicare patient’s rights. Obtains insurance authorizations needed for discharge to long-term care (LTC) facilities. Schedule insurance meetings with provider to obtain coverage for care transitions plan. Performs a variety of administrative duties including but not limited to maintaining the department schedule/on call schedule, obtaining needed office supplies, maintain multiple spreadsheets for data collection and other duties as directed by the Director of Case Management. Assures that a viable administrative support program is maintained at all times. Associate degree required in health care or related field.Good computer skills. Excellent verbal communication and interpersonal skills. Excellent telephone etiquette. Ability to prioritize tasks, use critical thinking, and problem solve. Ability to establish and maintain effective working relationships with patient/families/patient representatives, providers, and other health care personnel. Proficient skills in Microsoft office and Outlook tools.
