TheCare Team Specialistensures an accurate and timely referral process and insurance verification for services is carried out. Schedules patient admissions, evaluations, and routine appointments. Gathers all necessary referral paperwork and regulation for compliance. Initial point of contact for inquiries regarding the team from patients, families, staff, and other customers.
The Patient Access Representative:
* Verifies patient insurance and obtains precertification and/or preauthorization as appropriate.
* Obtains needed forms/information for referrals.
* Assists with various audits and compliance requirements.
* Communication liaison between the care team and the patient; including calls, electronic messages, paperwork and fax transmissions from patients, pharmacies, and other providers, to provide a timely response.
* Reviews referral documents and collaborates with clinical team for timely resolution of referral questions and concerns.
* Updates providers with referral concerns or questions.
* Responds to patient and provider questions regarding insurance process.
* Provides assistance to other team members when necessary.
* Coordinate electronic messages including obtaining additional pertinent information from the patient as well as answering items prior to sending to care team.
* Participates with PODs per POD charter, managing providers’ daily schedule of patient’s appointment and working closely with the rest of the care team to adjust as needed for optimum patient care.
* Huddle with the care team to ensure alignment/preparation with each patient during the day to optimize efficiencies.
* Educate patients to our workflows and process of referrals, or other services we provide.
* Maintain workflows including: new patient paperwork, no show policy is followed.
* High School degree or equivalent required
* Two years of related experience in a home health agency or other related experience preferred.
* Experience or education providing basic knowledge of medical terminology referred.
* Must stay current in knowledge base and meet VNH-mandated education requirements
* Minimum: $17.84
* Maximum: $26.92
